Diagnosis of PMS

Premenstrual syndrome is a western view of a common gynaecological problem. Syndrome means it is a group of signs and symptoms which typically occur together but for which there is no satisfactory explanation or known cause. However, shiatsu massage and TCM not only recognize this group of symptoms as a particular pattern of energy imbalance called liver energy stagnation, but also defines an aetiology for it, namely emotional stress as well as a treatment based on meridian theory.

From a western medical point of view, the diagnosis of PMS is problematic. Over 150 symptoms have been isolated but there is no agreement on these and no definitive tests to confirm diagnosis. To further complicate diagnosis, many of the symptoms are psychological. To rule out the possibility that the symptoms are psychological or psychiatric in nature as well as rule out the risk of misdiagnosis and mistreatment, the symptoms must be restricted to the second half or luteal phase of the cycle. As there is no significant difference in hormone levels between women with and without PMS, PMS seems to be due not to an hormonal imbalance but to a reaction to normal hormonal changes during the cycle.

Shiatsu Massage Treatment of Menopause

Shiatsu massage can help with the unpleasant symptoms related to premature menopause (premature ovarian failure or POF), perimenopause or menopause. The main pattern of imbalance here is liver blood-kidney yin deficiency. However, as there are several patterns of imbalance giving rise to menopausal symptoms, a number of different symptoms can be experienced such as hot flushes, sweating sleep disturbances, increased risk of heart disease and stroke, osteoporosis, thinning of skin, vaginal dryness, changes in the intensity of orgasms and libido, painful intercourse and having to empty the bladder more frequently.

Shiatsu massage practice is based on the oriental medical principle that mind and body is regulated by energy flowing along a network of meridians. According to this principle, the energy responsible for the functioning of mind and body flows through these meridians and a person becomes unwell once this natural flow of energy is disturbed. If this natural flow is restored, then the natural healing power of the body returns the person to good health.

These disruptions may stem from emotional factors, climate, lifestyle or other causes including stress, the presence of impurities in the environment or the body and physical trauma. These disturbances follow well-known patterns and can be corrected during shiatsu massage by applying pressure along the meridians with an emphasis at relevant pressure points. Before doing Panchakarma there are certain steps to be followed which are called as Poorvakarmas. They are Snehana (oleation) and Swedana (sudation). Snehana can be done by two ways, application of oil internally and externally. There are four types to promote sweat in Swedana. The purpose of Poorvakarma is to liquefy and guide the provoked Doshas (impurities) to the mainstream to facilitate the Sodhana karma. Diseases occur when toxins get accumulated in body and clog these channels. Unhealthy diet, lifestyle, stress, changes in environment etc lead to accumulation of toxins in body. These accumulated toxins have to be expelled out in order to keep our body healthy. Through Panchakarma therapy these toxic clogs are removed to normalize the physiological process. Hence Panchakarma therapy is called as detoxification therapy. Panchakarma therapy can be administered to a normal person to prevent diseases and keep his body healthy. Panchakarma therapy is also used as a purifactory therapy to cleanse the body before starting a treatment. A diseased body can be compared to a soiled cloth which can not be colored as we desire. Hence to attain the maximum benefits, the body has to be purified or cleaned before starting the treatment. The cleared channels help medicines to penetrate the deeper tissues. SNEHAN It is the oilation therapy. It is of two kinds, internal and external. Internal Snehan: in this treatment medicated oils and ghee (clarified butter) etc. are given for internal use. These sneha are so subtle that they can enter into minute pores of cell membrane. They have a tendency of pushing or mobilizing the impurities from there so that their elimination becomes easy. Ghee (clarified butter) prepared from cow’s butter is best for this use. External Snehan: this comprises of different kinds of massage. Medicated oils are applied on the body and different methods are used for that. These massage techniques are many times used in the treatment of muscular fatigue, loss of motor functions, arthritis, paralysis, migraine, spondylities and bodyache. They also induce sound sleep by relaxing the nervous system, increase immunity of the body and are beneficial to increase skin’s lustre. SWEDANA It is the second Purvakarma. It means fomentation or heating therapy. Different types of fomentations like steam bath and poultices are used on specific parts of the body. Many kinds of warm medicated oils, water, milk and herbal decoctions are also used. They are either used in a bath tub or are poured on the body. These types of fomentation treatments are very beneficial in diseases like pains in the joints, neck, lumbago, hemiplegia, urinary disorders, renal diseases, muscular dystrophy, common cold, asthma and loss of muscle functions. Swedana or fomentation liquefies or digests the Doshas and opens the mouth of blocked channels and pores of cell membrane. This makes the elimination of wastes easy when the actual Panchakarma are performed. So after doing these Purvakarma or preparatory treatments, the main karmas (Panchakarma) are done.

Von Willebrand Disease Treatment – Orthodox Medicine

von Willebrand disease Treatment (VWD) is based on the type of VWD and the severity of symptoms.

The aim of therapy is to correct the acquired clotting problem. This is mostly and usually accomplished by raising the levels of VWF (von Willebrand factor) and factor VIII (another protein in the bloodstream).

Most cases of VWD are mild, so you may need treatment only if you have planned or abnormal conditions such as minor surgery procedures, tooth extractions, or after an accident.

If your condition does require treatment, there are several options and medicaments available.

DDAVP® {Desmopressin acetate} Injection

This medicine is nothing but a modified synthetic copy of vasopressin (a natural hormone), which works by assisting the body to release stored VWF into the bloodstream side by side by increasing the levels of factor VIII.

Side effects may include:

-local injection (site reactions),

-facial flushing,

-headache

-changes in blood pressure.

It is mostly given to patients as an infusion (by means of injection into a vein), but it can also be given subcutaneously (injected under the skin).

Stimate® (Desmopressin acetate) Nasal Spray 1.5 mg/ml

Stimate® (Desmopressin acetate) in the form of Nasal Spray 1.5 mg/ml is another type of Desmopressin, made as a high concentrate nasal spray.

Plusses:

– It can be used easily without the need of help like injections.

Stimate usage is recommended for people with mild or moderate Type 1 VWD.

Common side effects:

-facial flushing

-headache

Note that Desmopressin nasal spray is fabricated in two forms:

-low concentrate

-high concentrate

Low concentrate nasal spray is used to treat:

– diabetes insipidus

-bed wetting diseases

High concentrate nasal spray is aimed for von Willebrand disease Treatment

Factor replacement therapies – Factor Concentrates

Factor replacement therapies are medicaments given to replace the missing VWF in patients with VWD.

There are two kinds of replacement therapy:

-factor concentrates containing VWF, (preferred for von Willebrand disease Treatment because they are treated to kill any possible viral infection)

-cryoprecipitate.

Both Factor Concentrates are derived from fresh plasma and are set to be given by infusion.

Humate-P®

Humate-P® the Antihemophilic Factor/von Willebrand Factor Complex (Human). Is the only VWF concentrate approved in the United States for von Willebrand disease Treatment (it is used for all types of von Willebrand disease),

Humate-P is effective for most if not all bleeding episodes in all types of von Willebrand disease.

Usage:

-Humate-P is taken by injection. (Home infusion).

Antifibrinolytics (Local clotting agents)

Typical local clotting agents, or antifibrinolytics, usually taken as an injection or via oral tract (by mouth) (tablet, capsule, mouthwash or elixir).

Role:

They help protect clots and hold them in place (anti burst action) thus they help evacuate the reasons of thrombosis i.e. they decrease the possibilities of brain insults, heart attacks…

Usage:

antifibrinolytics can be used to stop bleeding in many circumstances and specially those related to digestive system problems, bleedings from mouth, nose, or gastrointestinal tract.

They are highly used to help control heavy bleeding during the menstrual period.

Oral contraceptive pills (Birth control pills)

OCP, Birth control pills, or oral contraceptives, can increase the levels of VWF, by affecting the glandular system as a whole, side by side by affecting the tonus of the white muscles and their mighty role in powering the immune system and fighting hypertension, though they increase the risk of clotting, and is not favored in women with hypotension or thrombolytic diseases

Oral contraceptive pills should be used with the supervision of a doctor specially when it is taken for von Willebrand disease Treatment.

Oral contraceptives are very useful for women with HMB syndrome (heavy menstrual bleeding syndrome).
